Tax forms

The library will NOT be receiving any state tax forms. We currently have 2018 Federal 1040 instruction booklets but have NOT received any forms. You can order the state forms by phone at 1-888-728-2937 (leave a message) or send a written request for forms to PA Dept of Revenue, Tax Forms Service Unit, 1854 Brookwood Street, Harrisburgh, PA 17104-2244 or go online to www.revenue.pa.gov/PIT
